When choosing a plan, consider the total cost, including premiums and service costs. All plans cover preventive screenings fully but differ in deductibles, copays, and coinsurance. Medical Plan Comparison

Blue Shield CDHP Base and Buy-Up Medical Plans

The Blue Shield Base and Buy-Up plans let you choose between in-network providers at negotiated rates or out-of-network providers at a possible higher cost. Copays apply to select services, except in-network preventive care, which is fully covered.

Other services may require meeting the annual deductible and coinsurance. After reaching the out-of-pocket maximum, the plan covers all eligible expenses for the rest of the year. You’ll save the most by using in-network providers.

Kaiser HMO

The Kaiser HMO plan covers a wide range of health care services. You pay a copayment before the plan pays 100% for most services. You must obtain services exclusively from a Kaiser facility except in the case of life-threatening emergencies.

Prescription Drug Coverage

Prescription drug coverage is included in the medical plan you select. Regardless of which plan you choose, you’ll save the most money by using a participating pharmacy. You can access a list of pharmacies through your plan’s website or by calling member services.

High Deductible Health Plans: If you are enrolled in the base plans or Kaiser, the deductible applies to your prescriptions. Once you meet the plan deductible, you will pay the prescription copay. If you are enrolled in the buy-up plan, the deductible doesn't apply to prescription drugs, you will be responsible for the copay of the prescription drug.

Once you receive your new medical and prescription ID card, don't forget to let your provider know to ensure accurate and timely processing of claims.
