Benefits
As a regular, permanent Databricks employee located in Singapore, you are eligible to participate in the following group health plans as of your hire date:
- Cigna: Inpatient and Outpatient, Dental, Optical, EAP
- Singlife with Aviva: Term Life, Personal Accident, Critical Illness
If you are a new hire or are experiencing a life event (i.e. marriage or birth of a child), please complete this enrollment form within 30 days of hire or the life event date. Any delayed submissions will be subject to insurer medical underwriting.
You may elect coverage for yourself and the following eligible dependents, if applicable:
- Your spouse or domestic partner
- Unmarried child, stepchild or legally adopted child under the age of 18
- Unmarried child, stepchild or legally adopted child aged 18 to 26 who is in continuous full-time education
You will receive your Cigna Envoy® Member Portal information via email - you can review registration instructions and learn more about the Cigna app from page 35 of this document.
