Databricks offers medical coverage through UnitedHealthcare (UHC) and Kaiser (California Only).
When deciding which medical plan is the best fit for you and your family, it’s important to consider the total cost of coverage. This includes what you pay in premiums out of your paycheck and what you pay for services.
While each medical plan covers preventive screenings in full, the medical plans vary on annual deductibles, copays, and levels of coinsurance. This means you may pay more out-of-pocket costs with one plan versus another. The ideal medical plan should cover most of your health plan with out-of-pocket costs that meet your budget.
Importance of Getting Prior Authorization (UHC members)
Determining what’s covered by UHC ahead of time will help you avoid unexpected medical bills. To help mitigate this, you should obtain approval (aka prior authorization) from UHC before receiving certain medical services or treatment. With prior authorization, UHC agrees to help pay for the service (subject to any cost-sharing or other limitations).
Generally, if you are receiving services from in-network providers, your physician is responsible for obtaining prior authorization from UHC. But when going out-of-network, it is your responsibility to obtain the prior authorization. Without this approval, you may be responsible for the full cost of the service or treatment.
Examples of services where prior authorization is needed include:
- Non-emergent ambulance services
- Clinical trials
- Home health care
- Inpatient hospital services
- Major Diagnostic & Imaging (CT, PET scans, MRI)
- Injectable medications
- Reconstructive Procedures
- Rehabilitation Services (physical and occupational therapy)
- Outpatient surgery

Prescription Drug Coverage
Prescription drug coverage is included in the medical plan you select. Regardless of which plan you choose, you’ll save the most money by using a participating pharmacy. You can access a list of pharmacies through your plan’s website or by calling member services.
If you enroll in the UHC HDHP/HSA medical plan, you will pay the full cost for prescription drugs until you meet your deductible. Once you meet the plan’s deductible, you share the cost of prescription drugs with the plan.
Reminders
Your ID card will be mailed to the address listed in your Workday profile. If you have not received your ID card or need to request a new one, contact UHC at 855-251-6690. You can also obtain an electronic version via myuhc.com or the UHC app. Once you receive your new medical and prescription ID card, don't forget to let your provider know to ensure accurate and timely processing of claims.
