PPO Dental Plan
With the Delta Dental PPO dental plan, you may visit any dentist of your choice. Keep in mind, you’ll receive the highest coverage when you use an in-network provider. If you visit an out-of-network provider, you will not benefit from discounted rates and will pay more out-of-pocket for services. Many dentists outside the provider network will require you to pay for services upfront and submit forms for reimbursement from your plan.
Delta Dental Plan Maximum Enhancement
120 days after the previous calendar year ends, the system automatically applies a rollover benefit of up to $600 from any unused annual maximum. This enhancement works as follows:
- Claims are first applied to the current year’s standard annual maximum.
- Once the standard maximum is met, claims are then applied to any rollover enhancements.
- Rollover enhancements are used in chronological order, starting with the oldest.
Qualification
To qualify for the Maximum Enhancement (rollover) in a given calendar year, an enrollee must submit at least one claim for covered dental treatment during that year. Failure to do so will result in ineligibility for that year’s enhancement.
- Enrollee’s will retain all previously granted enhancements.
- Enhancements must be utilized within two years from the date they are earned.
- The total accumulated Maximum Enhancement cannot exceed $1,500.
